Requirements for New Mentors

1. Live Scan Background Check

Stanford’s policy for Protection of Minors requires that anyone working directly with, supervising, chaperoning, or otherwise overseeing minors (individuals under 18 years of age) in Stanford-sponsored programs or activities must complete a Live Scan background check.

If you are participating as a mentor in FAST, you must complete a Live Scan background check. A few times a year the university offers free Live Scan events, but it is very sporadic. A list of Live Scan locations nearby can be found here. The nearest one to Stanford is the UPS Store in Town and Country.

Once the Live Scan is submitted, please submit this form to Stanford and this form for our bookkeeping.

2. ESUHSD Volunteer Form

Please fill out the volunteer form for the appropriate school and submit it along with a photo of your ID to the Google Form. The forms can be found here.

3. Complete Mentor Training

Dates TBD!

4. STARS Course: WWM-1000-WEB Working with Minors at Stanford

This course also needs to be completed. It can be found in Axess > STARS > CATALOG > Search Catalog > WWM-1000-WEB in the search bar.
